Event Request Forms
Thank you for being a part of our vibrant community!
Interested in planning an event? We have three different event request forms to make the approval process easier depending on the type of event.
- For events that are sponsored by the Administration Team, such as Christian Service Ministries or for parish-wide events, please use the Administration Team Form.
- For events that are sponsored by the Faith Formation Team, including Religious Education, Youth Ministry, or Adult Faith Formation, please use the Faith Formation Team Form.
- For events that are sponsored by the Liturgy Team, including Music and Christian Worship Ministries, please use the Liturgy Team Form.
Please submit all requests at least two months in advance of the event. The staff team will discern the request and respond within two weeks of submission with approval or additional questions. Please note that because we are a growing, vibrant parish with many events at any given time, not all events may be approved. We appreciate your cooperation in completing the whole form in as much detail as possible. Thank you for all you do!
